Teams of 5 (Up to 3 additional substitutes may be rostered)
$20 per team

Gatorade, snacks and pizza provided
Return form and entry fee to Mike Supp, Mr. Vincent, or Mr. Hollenbach or to Rm. 110 by Wednesday February 1st
